There were four men huddled on the bow of the burning, 190-foot fishing vessel Galaxy. Captain Dave Shoemaker was seriously hurt, with internal injuries and deep tissue burns to his hands, arms, legs and torso. After several desperate attempts to enter the smoke-filled bridge, find a radio and issue a mayday call, he had climbed onto the roof of the wheelhouse in an effort to quell the panic that consumed the twenty-two surviving crewmembers aboard his vessel.

Eighteen men and one woman had fled the heat and smoke and mustered atop the shelter deck at the stern, 34 feet above the water, where they watched the advancing flames that flared through holes emerging in the steel deck plates. There were three more men on the foredeck, isolated from the others, when Shoemaker fell.

The captain had been struggling to raise a bundle of survival suits the trio on the foredeck had lashed together using a three-eighths-inch polypropylene line so he could distribute them to the crewmembers on the stern when an explosion rocked the boat and sent him tumbling over the safety railing. He screamed as a four-inch diameter pipe broke his twenty-foot plunge, shattering three ribs, knocking his shoes off and leaving him lying on the blistering hot steel.

I remember laying there and it sounded almost like bacon frying in a frying pan. It was just sizzling the skin, and I knew I had to get out of there so I started rolling. I kept saying, ‘my crew, my crew, they need to get into the raft and I’m not there to help them.’

Captain Dave Shoemaker’s gripping account of the fire that consumed his vessel on October 20, 2002 ranks among the great stories of tragedy and courage at sea. He was alone in the wheelhouse shortly after 1600 hours when one of his processing factory foremen burst into the wheelhouse and shouted that there was smoke in the fish factory.

And I said, what type of smoke is it, and he said, we’ll it’s like that smoke behind you.

Shoemaker turned and what he saw brought him to his knees. Already, the wheelhouse was filling with thick black smoke. Within four minutes, the vessel would be rocked by the first of many explosions, a blast that hurled three members of the starboard fire team through the three-by-three-foot gear-setting hatch at the stern and 30 feet through the air, into the frigid waters of the Bering Sea. Crippled, without power or steerage in the fifteen-to-twenty foot seas and 30-knot winds, forced out of the wheelhouse by the intense fire that roared out of control below decks, without radio communications or any means of calling for help, with men in the water, with the port life raft consumed in flames and survival suits melting on the blistering deck plates, Shoemaker struggled to save his crew mates and himself. For his efforts in saving twenty three of the twenty six men and women aboard the Galaxy that day, Shoemaker would be awarded the U.S. Coast Guard’s Gold Lifesaving Medal, the agency’s highest civilian commendation for heroism.

Now, the veteran of twenty-five years at the helm of Bering Sea fishing vessels has dedicated himself to sharing with fellow seamen the lessons learned during his harrowing ordeal. This is an educational DVD that will make your crew sit up and take notice. Modern ships are extraordinarily seaworthy craft, built to exacting standards of construction and outfitted with technology worthy of the Starship Enterprise. Even the best ships can capsize or sink with terrifying speed, however, and equipment is only as good as the men and women who operate it.

These realities are reflected in the Standards of Training, Certification and Watchkeeping for Seafarers (STCW) that have been adopted by the International Maritime Organization (IMO) and its member nations. Competence in Personal Survival is one of four core elements of the Basic Safety Training that all ships are now required to provide, and all seamen are obligated to achieve.

Using this Product

Intended for use by commercial marine industry and seagoing recreational vessels, this training program is based on requirements set forth in Table VI of the STCW Code, as ammended in 1995. Consisting of DVD training media, a Test cassette and a Workbook, the product offers training, testing and assessment features that facilitate compliance with the International Safety Management (ISM) and STCW codes.

Using this Product

Crewmembers should be assigned to watch Personal Survival, Parts 1, 2 & 3, and then view the audio-visual test while recording their answers to the test questions.

Answer sheets and Certificates of Completion should be signed by each crewmember, his or her supervisor and by the captain, and maintained with crew records. Crewmembers are encouraged to keep copies of the Certificate of Completion with their personal records as well.

Photocopy master versions of the answer sheet and Certificate of Completion can be found in the back of the Workbook. Remove these pages and create as many copies as you require. You also have the option of removing the answer sheet at the back of the Workbook before it is issued to crew personnel. VHS refers to videotape. NTSC is the U.S. media format and is used in countries including Canada, Mexico and Japan. PAL is the European media format used in more than 85 countries.
